The Challenge We’re Addressing
Healthcare costs are 2.5x higher for people experiencing homelessness, mostly driven by preventable Emergency Department visits and hospital readmissions. Why? Because they are trying to heal from an injury, an illness, or a medical procedure while on the streets.
Shelters do not provide a safe, recuperative place to heal. The impact from trying to heal on the streets time and time again is debilitating and just exacerbates the cycle.

Our Solution: Health2Home
Health2Home stabilizes unhoused individuals after a medical event through a safe, recuperative place to heal, clinical service provision, community care connection, and a guided path to stable housing. This program helps to break the cycle by addressing both short-term recuperation and long-term stable housing.
This is a new emerging model across the country to help address the rise in homelessness—especially within the senior population—a rise in inflation including healthcare costs, and a lack of affordable housing options. This model can be referred to as medical respite or recuperative care.

How Health2Home Works
Through our medical recuperative care program, clients receive:
- A bed they can access 24/7
- 3 meals a day
- Time and space to heal
- Daily medical and mental health wellness checks
- Case management
- Housing placement support providing a guided path to stable housing
Additionally, supports can be provided on an outpatient basis for those who do not need a bed but do need a guided path to stable housing.
