By completing this form you are requesting to purchase tickets for the Call Sam Chopper Shop Raffle. After filling out the form you will be contacted to complete your purchase via phone(We will call from (248) 791-5111). Please note: staff are only available Monday through Friday from 9 am to 5 pm. Should we miss your call, we will call back as soon as we are able.

Tickets will be able to be purchased at the following prices:
$20 for 1 raffle ticket
$40 for 3 raffle tickets
$100 for 8 tickets.

Please note any employee of Volunteers of America Michigan, The Sam Bernstein Law Firm, Bad Pig Customs, or any person who worked on the custom chopper is prohibited from purchasing a raffle ticket.
